Hole covers are cleated and/or secured in place.
If an Okland employee creates a hole in a walking/working surface, the Okland supervisor overseeing the work shall ensure that the hole is covered and the cover is maintained. Hole covers shall be properly constructed, marked, and secured from accidental displacement.
